Wound Care:
Our nurses will assess and manage a wide variety of wounds including diabetic ulcers, pressure ulcers, post-surgical wounds, and manage wound VACs.
Medication Teaching and Management:
Our nurses will educate patients on their medications regarding drug interactions, safety concerns, side effects, monitoring, and compliance importance.
Foley/Ostomy Maintenance:
Our nurses will assess, manage, and provide education to patients and caregivers regarding their foley/ostomy care.
IV Therapy/Management:
Our nurses will assess, manage, and provide education regarding IV therapies.
Patient and Caregiver Education:
Our nurses will develop a unique plan of care tailored to the specific needs of each patient, providing them and their caregivers with education and safety prevention strategies in collaboration with their physicians.
Chronic Disease Management:
Our nurses will develop a plan of care that will assist the patient and their caregivers in the management of their chronic disease process. The plan of care will be tailored to the specific needs of the patient and will be created in collaboration with their physician. Our nurses will manage a variety of disease processes including but not limited to Diabetes, Hypertension, CHF, and COPD.
